Privacy Notice

This notice explains how your information is collected and used when you complete the EMDR Supervision Feedback and Future Support Questionnaire.

1. Who is responsible for your information

HealthyMinds4u (Lynne Douglas, EMDR Consultant and Supervisor) is responsible for your information. If you have any questions about this notice, you can contact me at lynne@healthyminds4u.co.uk.

2. What information is collected

The questionnaire collects:

Your full name (so I know who has responded).
Your email address and preferred pronouns, only if you choose to provide them.
Your responses to the questionnaire questions.

3. How your information is used

Your responses are used to review and develop EMDR supervision and related support, learning, and wellbeing opportunities. If you provide an email address, it may be used only to contact you about your feedback or future opportunities. Your information is not used for marketing and is not sold or shared with any third parties.

4. Legal basis for processing

Your information is processed under the UK General Data Protection Regulation (UK GDPR) on the basis of my legitimate interests in reviewing and improving the supervision and support I offer, and with your consent where you choose to provide optional information such as your email address.

5. Client information

Please do not include identifiable client information, clinical case details, or any confidential information about clients in your responses. If such information is included, it will not be used for any purpose.

6. How your information is stored

Your responses are stored securely and are accessible only to authorised people involved in managing this questionnaire and supervision service. Your information is kept only for as long as is needed to review and act on your feedback, after which it is securely deleted.

7. Your rights

You have the right to request a copy of the information I hold about you, to ask for it to be corrected or deleted, and to withdraw any consent you have given at any time. To exercise any of these rights, please contact me at lynne@healthyminds4u.co.uk.

If you are not satisfied with how your information is handled, you also have the right to complain to the Information Commissioner's Office (ICO), the UK's data protection regulator.
